This is a tool to track response to medication and will be used to adjust the doses and timing of medications.

Please place only one check mark under each "Time of day" column, in the row that best describes the patient's motor state over the 1 hour period prior to the time indicated (ie: in the 7:00 AM column indicate the average motor state from 6:00 AM to 7:00 AM or if asleep check only the "asleep" row.
